Melatonin

Melatonin lowers body temperature and decreases alertness, and suppresses thyroid and progesterone. Most social, occupational, and academic settings, due to incompatibility of their internal biological rhythms with the environmental timetable. Consider, for example, a patient with DSPS who is expected to arrive at his workplace by 8 or AM. In order to fulfill this requirement, this individual is forced to wake up at what might be the middle of his internal night. It is not surprising, therefore, that he will be frequently late and or absent, a pattern that will most likely subject him to disciplinary actions up to dismissal. If, however, he manages to meet the attendance standards, his performance will be liable to the detrimental effects of sleep loss and time of day. In childhood and adolescence, when CRSDs usually emerge, the impairment of daytime functioning can be even more remarkable than in adults. Unlike adults, who can at times choose a lifestyle that corresponds to their sleep-wake cycle, the activity hours of persons of younger age are constrained by a strictly predetermined school timetable. The inability to adjust to this timetable may be associated with deteriorated school performance. In a recent study, we found that the vast majority of young patients with DSPS complained of frequent late arrivals and absences at school, underachievement, and behavioral social difficulties. Importantly, treatment with melatonin significantly reduced the number of children and adolescents complaining of malfunctioning at school.63 In some cases, the daytime functional difficulties might be severe enough to be mistakenly interpreted as symptoms of psychiatric disorders. A case of a 14-year-old boy provides a dramatic illustration of such a scenario.64 During the 4 years prior to his referral to our sleep clinic, the patient suffered from major functioning difficulties, including conflicts with teachers, parents, and peers. At the age of 12, the patient dropped out of school and was sent to an inpatient child-psychiatry center. Three months of psychiatric evaluation yielded diagnoses of atypical depressive disorder with possible schizotypal personality disorder. Due to excessive daytime sleepiness, he was referred to our sleep clinic for assessment of a potential sleep disorder. A thorough sleep study revealed that the patient had a 26-h sleepwake schedule and dissociation between oral temperature and salivary melatonin rhythms. He was diagnosed with non24-h sleep-wake pattern. Treatment with oral melatonin normalized the sleepwake schedule within a month, and follow-up actigraphy. FIG. 1. The contour plot is the result of Kriging analysis of growth index data. The melatonin scale has been transformed to facilitate the response surface modeling by making a power transformation of 0.15. The bucket shape of the response surface indicates a synergistic interaction between INH and melatonin on mycobacterial killing.

FS.3 Innovative Teaching Project: Short Movie "A Medical Appointment" Fabiola Irisarri, Rut Berdn, Ana Hospital, Beatriz RodriguezVega, Elena Fernandez-Leon, Marta Morales La Paz University Hospital, Spain Our project tries to show different perspectives of a common situation: a medical appointment. We use to short films. On the first one, two actors represent a doctor and a patient. Both characters emphasize critical aspects of a doctor-patient relationship. We write this script based on the two patient's real experiences which were included on the second short-film together with movies sequences. On the second short film we elaborate different doctor's views, adding a more subjective and personal experience than the first one, and at the same time we keep cinema perspective. This narrative tool offers an enlarged picture from the distance of a common situation. These two short films are included in a innovative teaching project with medical students of the Autnoma University of Madrid. After the projection, a discussion about doctor-patient relationship was established. Our objective was to encourage a critical approach to this topic. Short films length of time First film: 4 minutes, Second film: 8 minutes Real patients gave consent to be film and to project their testimonies. FS.4 Restore Hope Selda Asal1, Kemal Sayar2 1Istanbul, Turkey 2Bakirkoy Training and Research Hospital, Istanbul, Turkey Hope is a concept that shapes our thoughts about daily life. An approach that accepts as normal people belief in a future, and that this future may bring alternatives to the current situation. To wait for something to happen within the normal flow of life, and to feel that life has a meaning is also a kind of hope. But when hope looses its position, disappears, or changes its position with playing with disappearing, then what happens? In the two films Love is fake, Compiting with genies ; Suicide or drug abuse as a different version of suicide as a solution to feeling trapped, without alternatives. These two films are made up of the animated drawings and paintings made by girls and boys at Bakirkoy Mental Clinic in Istanbul. The girls, admitted for suicide attempts, the boys for drug abuse. You have to start anew. That is where this film begins. On the banks of the river Danube, surrounded by the beautiful landscape of Upper Austria lies the picturesque town of Mauthausen. Two kilometres from its town centre is a place that attracts busloads of tourists, parties of schoolchildren, people from all over the world. Tour guides come to work here everyday and metaproterenol. Intensity of the EMF from a bright display is much lower than that used in these laboratory studies, it is thought that the EMF from the bright display had little effect on melatonin level. In conclusion, performing an exciting VDT task with a bright display influences the nocturnal melatonin concentration and other physiological indicators of the human biological clock. Given the diversity of effects caused by melatonin and the complexity of its actions, long-term changes in this hormone might have wide ranging consequences for health. It has been proposed that these might include altered incidence of solid tumours or leukaemias, as well as effects on ageing and neurodegenerative diseases. While these endpoints are of potential interest, most melatonin-related EMF research has been focused on breast cancer. As part of a previous comprehensive evaluation of the potential of power frequency EMFs to cause cancer, the Advisory Group on Non-ionising Radiation AGNIR ; concluded that melatonin rhythms were not substantially affected by exposure to magnetic fields AGNIR, 2001 ; , although the preliminary data from one investigation suggested some effects may occur, possibility in a sensitive subgroup of the study population. Since the publication of that report, further studies have been published relevant to an understanding of possible effects of EMFs on melatonin. As a consequence, the Board of the NRPB asked the Advisory Group to examine the evidence related to effects of EMFs on the production of melatonin and to advise on whether this could be implicated in the development of breast cancer and methoxsalen.

Sustained a fracture are at increased risk of subsequent fracture. Osteopenia cannot, however, be accurately diagnosed from plain radiographs, and the presence of radiological osteopenia in these instances should lead to a more formal assessment of bone mass. BMD is usually measured by dual-energy x-ray absorptiometry DEXA ; of the spine or hip. In the over 65s, spinal measurements can be artifactually influenced by the presence of degenerative changes and aortic calcification. In older subjects therefore, hip BMD is recommended. Measurement of BMD at one skeletal site is generally most predictive of fracture at the site, although fracture risk at any site can still be assessed.16 This has resulted in the use of machines that are cheap and portable to measure BMD at peripheral sites such as the forearm and calcaneus. Quantitative ultrasound offers an assessment of bone microarchitecture that is independent of BMD, 17-20 and studies have demonstrated that ultrasound is able to predict fracture risk. These techniques of peripheral bone densitometry and quantitative ultrasound can be utilised to aid in fracture risk assessment and to identify patients who may benefit from therapy and montelukast.

The process of converting a new chemical entity into a marketable product involves a great deal of research and investment. During this research process a number of new and potentially patentable discoveries may result. These discoveries may result in "secondary patents", the aim of which is generally to extend the period of patent protection of the compound and therefore extend the period of exclusivity during which the product can be marketed free from generic competition. Possible types of secondary patent include: Composition patents Patents for new polymorphs Patents for new formulations Synthesis patents Patents for new therapeutic uses Patents for new treatment regimes Patents for metabolites pro drugs. Cle of Willis on the base of the brain is incomplete. Thus, in the absence of collateral circulation the forebrain can be deprived totally of oxygenated blood by temporarily constricting the carotid arteries bilaterally. Again, in this species melatonin reduced damage to the brain resulting from temporary interruption of blood flow in the carotid arteries. In each of these reports, the authors attributed the beneficial effects of melatonin to its free radical scavenging and ancillary activities. Even fetal brain injury after occlusion of the ovarian arteries in pregnant rats has been shown to be reduced when melatonin is administered to the mother Wakatsuki et al., 1999 ; . Melatonon is known to readily cross the placenta Okatani et al., 1998 ; . In addition to its efficacy in reducing brain injury after hypoxia reoxygenation, melatonin has been used in studies with other organs where blood flow was transiently interrupted. Of particular note is melatonin's ability to reduce the severity of cardiac damage after a period of ischemia followed by reperfusion, i.e., heart attack. Tan and co-workers 1998b ; found that melatonin was 30 times more effective than vitamin C in reducing the cardiac arrhythmias associated with interruption and then restoration of blood flow to the heart. Similarly, Lagneux and co-workers 2000 ; described melatonin to impart spectacular protection against both the irregular cardiac contractions and cardiac lesion size after ischemia reperfusion. Finally, in the retina Osborne et al., 1998 ; , liver Fig. 9 ; Sewerynek et al., 1996a ; and gut de la Lastra et al., 1997; Cuzzocrea et al., 2000b ; melatonin reduced the loss of tissue and the compromised function that followed hypoxia reoxygenation. In these collective studies many different parameters of cytotoxicity, molecular damage and function were measured; without exception, all benefited when melatonin was administered. As in the brain, no dose-response relationships have been established nor has the window of efficacy for melatonin been defined in any of these organs. There are now numerous published studies were melatonin has been used to combat oxidative damage in other situations. Space limitations preclude a discussion of all these studies in this brief review. Only a few of the additional reports will be mentioned here. Hence, melatonin has been effectively used to negate what is believed to be free and metaproterenol.

Similarly, the pineal gland and its principal secretion, melatonin, are known to influence the initiation and progress of cancer. Internalized the hMT2 melatonin receptor, but not the hMT1 melatonin receptor, through an arrestin-dependent mechanism. We conclude that the differential regulation of the hMT1 and hMT2 melatonin receptors by short melatonin exposure may be a mechanism by which the hormone regulates temporally-mediated physiological events in vivo.
